If your organization is state tax exempt, you must submit
a completed IRS 3372 form prior to arrival.
Email the completed form to scrreservations@shantycreek.com (or it fax to 231-533-7004). Be sure to note the name of the person on the lodging reservation associated with this form in your email or fax.
